This one covered every possible type of costume piece for EPCOT Center (note items such as "Obi" and "Ascot", which would have been worn in World Showcase). After each shift, Cast Members turned in their costumes, recieved a fresh costume, and a Wardrobe Cast Member filled out one of these forms.
